Please make sure you have the appropriate drug testing Custody & Control Form (multi-part drug testing form) for the location you plan on using.

Contacting the Employee Assistance Program (EAP)

When a participating employee tests positive, refuses to be tested, or violates the rules of the PPSAP drug and alcohol testing program, he/she must be immediately suspended and instructed to contact the Substance Abuse Professional (SAP) providing the EAP services for an evaluation and assessment. There is no charge to the employee for this service.
